1. Signup - Signup for a patient portal account here.
2. Login - Login and begin filling out the online forms.

After the online forms are completed you will be contacted to discuss the next steps in accessing care at Thoughtful House. It is important to note that the clinicians that work with Thoughtful House provide individualized care; only after the Clinical Care Department has reviewed the information gathered on these online forms will an appropriate plan of care be determined.

Should additional information be needed, you will be contacted to complete a call with an Intake Coordinator. This call typically lasts approximately 30 minutes. After this information is shared with the Clinical Care Department the appropriate referrals will be made.

Considerations in scheduling will be made for families traveling from out of town that have been referred to see more than one clinician.

This process is designed to make initial consults with the various clinicians more efficient and allows us to provide a higher standard of care.
