Are you interested in services at Thoughtful House? If so, the first step is to complete the new patient intake forms. Click on the link below and follow the instructions to complete these forms. Please complete them as thoroughly and accurately as possible.
- Signup - Fill out the form https://my.thoughtfulhouse.org/signup.php.
- Confirm - Within minutes you will have an email. Click on the link in the email.
- Login - Login and begin filling out the intake forms.
After the intake forms are completed you will be contacted to discuss the next steps in accessing care at Thoughtful House. It is important to note that the clinicians that work with Thoughtful House provide individualized care; only after the Clinical Care Department has reviewed the information gathered on these intake forms will an appropriate plan of care be determined.
Should additional information be needed, you may be scheduled for an intake call with a Family Care Coordinator. This call typically lasts up to one hour. After this information is shared with the Clinical Care Department the appropriate referrals will be made.
Considerations in scheduling will be made for families traveling from out of town that have been referred to see more than one clinician.
This process is designed to make initial consults with the various clinicians more efficient and allows us to provide a higher standard of care.
